Tablelands Community Support

Tablelands Community Support  is auspiced by Uralla Shire Council, funded by the State and Australian Government and provides services to people who are aged or have a disability, and to their carers who live in the New England area.

Uralla Shire Council employs the staff of Tablelands Community Support (TCS)  to provide the day to day service management and case management for  clients.

Any one can make a referral  to TCS.  You may make the enquiry yourself or ask your family, friends or your Doctor to make the referral on your behalf

On receipt of a referral eligibility for a service  is determined and referrals prioritised and you will be notified of the outcome. A Case Manager from Tablelands Community Support  will then do a home visit  and discuss with you the type of services you require. They will develop a Care Plan with you, arrange the services you require and visit with you regularly to ensure the service is continuing to meet your needs.

Tablelands Community Support manages the following services

Tablelands Community Options
Provides comprehensive assessment and case management to assist the aged, people with a disability and their carers who wish to live in their own homes, to access HACC services.

Tablelands Community Options may be able to assist you if you:-

  • Require a Case Manager to act as your advocate if you are having difficulties with your services.
  • Require information on services that are available to you in the community
  • Need assistance in organising the services you require
  • Need assistance because existing services are not available or unable to meet your needs
  • Your carer needs a break
  • Require ongoing monitoring of your services to ensure they  are continuing to meet  your changing  needs

This program is available in the LGA’s of Uralla, Guyra, Walcha, and Armidale Dumaresq

Tablelands Community Support Options – Aboriginal
Identified funding is received to provide Community Options services to the Aboriginal community in the LGA’s of Uralla, Guyra, Walcha, and Armidale Dumaresq.  Aboriginal staff are employed to provide this service.

Community Aged Care Packages
Community Aged Care Packages are funded by the Australian government and are designed to support elderly people who wish to remain in their own home in familiar surroundings rather than move to residential care.

Before being accepted into this program, a member of the Aged Care team will visit you to determine your eligibility for this program. A Case Manager from Tablelands Community Support Options will then do a home visit to help you plan the assistance you need, record what you have decided and arrange the care you need and provide ongoing monitoring of your services

Services are available in the local government areas of Uralla, Guyra, Armidale Dumaresq, Walcha, Parry part B,  Quirindi and Barraba.

Tablelands Respite For Carers Of People With Dementia
An in home/out of home respite service for carers of people with dementia.

You may already be receiving some support from another service to assist you in this caring role.

Tablelands Respite for Carers of People with Dementia can provide extra assistance for you to take a break. 

Each service is tailored very flexibly to meet the individual carers needs.

Special funding is available to assist carers in times of crisis such as sudden illness, family crises and when the caring role becomes overwhelming.

The  service is  available   to  people  living in  the  local  government  areas of Uralla, Walcha, Armidale Dumaresq and Guyra

Tablelands Rural And Remote Respite Service:
Receives funding from the State Government to provide a recreational respite service for people with a disability aged 16 to 65 years who have a permanent disability and live in the LGA’s of Uralla, Guyra, Armidale Dumaresq or Walcha.
The Rural and Remote Respite  Service aims to assist a person with a disability to enjoy social activities in the community. 

This may be:-

  • a holiday
  • a regular outing
  • a weekend away
  • visit friends
  • anything else that you would enjoy that would be difficult to do on your own 

Narwon Elders Group
The Elders Group brings together the Elders from Armidale and Uralla to share a meal, friendship, discussions and outings.

The group is held each Tuesday from 10.30am to 2pm. It is held one week in Armidale at Kent House and the next week in the Community Centre in Uralla. The group also meets regularly with the Elders living in other local communities.

Transport can be arranged for people wishing to access the service. Carers are  available for those requiring extra support at the group.

Aboriginal staff are employed to provide this service.
